Job Description:

We are seeking a highly skilled and experienced Disability Support Specialist to join our team. As a Disability Support Specialist, you will provide personalized support to an individual with daily tasks, outings, and appointments.

Key Responsibilities:

  • Develop and implement personalized support plans to meet the individual's unique needs and goals
  • Provide assistance with daily routines, including meal preparation and household chores
  • Accompany the individual to medical appointments, social events, and other activities, providing transportation and emotional support
  • Maintain open and effective communication with the individual's support network, ensuring their needs are met

Requirements:

  • Strong knowledge of disability support principles and practices
  • Excellent communication and interpersonal skills
  • Ability to work flexibly, including evenings and weekends
  • Reliable and discreet
