About the Role:
In this role, you will conduct assessments and develop care plans tailored to the unique needs of service users. You will work collaboratively with colleagues, community organizations, and service users to ensure effective support and outcomes. Your expertise will be essential in promoting independence and empowering individuals to make informed decisions about their lives.

Key Responsibilities:
- Conduct assessments and develop care plans for adults with sensory impairments
- Build relationships with service users, families, and carers
- Engage with community organizations to promote access to local services and support
- Participate in team discussions and contribute to the development of best practices

Requirements:
- Registration with Social Work England
- Enhanced Disclosure application to the Disclosure and Barring Service
- Full UK Driving Licence (disability adjustments considered)
- Proficiency in spoken English/Welsh
