We are seeking an experienced Occupational Therapist to join our team at Pennine Care NHS FT, working with older adults in a mental health setting. As a key member of our multidisciplinary team, you will play a crucial role in delivering high-quality care and support to individuals with complex needs.
Key Responsibilities- Embed the Occupational Therapy assessment and intervention pathway across the older peoples' community mental health service, working collaboratively with the wider multidisciplinary team.
- Conduct comprehensive assessments, develop outcome-based care plans, and facilitate the implementation of agreed plans of care, utilizing evidence-based outcome measures.
- Provide a range of highly skilled interventions on both a group and individual basis, sharing expertise with other team members and developing treatment plans and interventions to be delivered by the team's technical instructor.
- Work effectively as part of a multidisciplinary team, contributing to discharge planning, assessments, and decision-making processes.
- Develop and maintain strong relationships with patients, carers, and other professionals, providing professional expertise in the care of mental health service users.
- Diploma or degree in Occupational Therapy and registration with the Health and Care Professions Council (HCPC).
- Substantial post-registration experience working in a mental health service, with a focus on older adults and complex needs.
- Experience of working with people with severe and complex mental health needs, including those with a range of mental health conditions and their impact on occupational functioning.
- Strong understanding of current local and national policies in relation to mental health, including the Mental Health Act, Risk Assessment and Management processes, and CPA processes.
- Excellent written and verbal communication skills, with the ability to record relevant documentation to a professional standard.
- Ability to work effectively as part of a multidisciplinary team, manage and prioritize own workload, and demonstrate specialist expertise in assessment and intervention.
