Job Overview

We are seeking a highly motivated and dedicated Support Worker to join our team in providing specialist case management and rehabilitation services to children and adults across North East England, North West England, and Scotland.

Key Responsibilities
  • Deliver high-quality support and care to clients, promoting independence and well-being.
  • Work collaboratively with multidisciplinary teams to develop and implement personalized care plans.
  • Provide occupational therapy and physiotherapy services to clients, enhancing their functional abilities and quality of life.
  • Contribute to the development and maintenance of effective case management systems, ensuring seamless service delivery.
  • Participate in medico-legal services, providing expert testimony and supporting clients through the legal process.
Requirements

To be successful in this role, you will require:

  • A strong understanding of case management principles and practices.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and colleagues.
  • Ability to work independently and as part of a team, prioritizing tasks and managing multiple responsibilities.
  • Flexibility to work part-time, with a minimum of 22 hours per week.
  • A salary of £13.00 per hour.
