Key Responsibilities- Deliver high-quality support and care to clients, promoting independence and well-being.
- Work collaboratively with multidisciplinary teams to develop and implement personalized care plans.
- Provide occupational therapy and physiotherapy services to clients, enhancing their functional abilities and quality of life.
- Contribute to the development and maintenance of effective case management systems, ensuring seamless service delivery.
- Participate in medico-legal services, providing expert testimony and supporting clients through the legal process.
To be successful in this role, you will require:
- A strong understanding of case management principles and practices.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and colleagues.
- Ability to work independently and as part of a team, prioritizing tasks and managing multiple responsibilities.
- Flexibility to work part-time, with a minimum of 22 hours per week.
- A salary of £13.00 per hour.
