About the Role

Job Summary

The Population Health Strategist will play a critical role in providing health improvement planning services in the local health district. This includes planning, implementing, and evaluating health education activities, as well as working closely with others to fulfill health improvement needs and goals.

Main Responsibilities
  • Health Improvement Planning: Develop and implement comprehensive health improvement plans to address the unique needs of the local community.
  • Grant Management: Prepare and submit grant applications related to health improvement projects, ensuring timely submission and successful funding outcomes.
  • Educational Programs: Design and deliver educational programs to clients and staff, incorporating cutting-edge research and best practices in health education.
  • Data Collection and Reporting: Collect program-specific data and write reports to demonstrate the impact and effectiveness of health education initiatives.
  • Needs Assessments and CHIP Process: Conduct thorough needs assessments and facilitate the Community Health Improvement Plan (CHIP) process to identify priorities and drive meaningful change.
Requirements
  • Education and Experience: A bachelor's degree in public health, health education, or a related field, along with considerable experience in health education, community health assessment, and improvement planning.
  • Grant Writing and Management: Proven track record of successfully writing and managing grants related to health improvement projects.
  • Communication and Interpersonal Skills: Excellent verbal and written communication skills, with the ability to effectively manage interpersonal and interorganizational communication networks.
  • Technical Skills: Proficiency in Microsoft Office products and ability to learn new software applications quickly.

As a Population Health Strategist at the Commonwealth of Virginia, you can expect a competitive salary of approximately $65,000 - $85,000 per year, depending on location and experience.
