Job Summary

The Hollymoor Medical Centre is seeking an experienced Advanced Nurse Practitioner to join our clinical team. As an integral part of our multidisciplinary team, you will be responsible for providing high-quality patient care, working collaboratively with GPs and other healthcare professionals to deliver evidence-based care.

Main Responsibilities
  • Assess, diagnose, plan, implement, and evaluate treatment plans for patients presenting with minor illnesses and undifferentiated diagnoses.
  • Triaging patients via online forms or telephone conversations, recognizing the need to assess patients at the most appropriate place to take into account their healthcare needs.

  • Assessing, diagnosing, planning, implementing, and evaluating interventions/treatments for patients with complex needs, proactively identifying, diagnosing, and managing treatment plans for patients at risk of developing long-term conditions.
  • Providing timely, high-quality, person-centered clinical care to patients, addressing their needs to the highest standard of evidence-based clinical care.
  • Following up patients as required, making and receiving referrals, and actioning results of investigations and medication requests.
  • Prioritizing health problems and intervening appropriately to assist patients in complex, urgent, or emergency situations, including the initiation of effective emergency care.
  • Supporting patients to adopt health promotion strategies that promote healthy lifestyles and applying principles of self-care.
  • Making professionally autonomous decisions for which you are accountable.
  • Booking diagnostic investigations as indicated.
  • Referencing patients directly to other services/agencies as appropriate.
  • Following up patients as required by clinical need and guidelines within the scope of practice.
  • Undertaking the assessment of pathology reports, interpreting results, and directing further action as warranted.

Requirements
  • Experience of using prescribing qualification and carrying out medication reviews within skill set and competencies.
  • Clinical examination skills.
  • Able to triage patients.
  • Communicates effectively with patients and carers, recognizing the need for alternative methods of communication to overcome difficult levels of understanding, cultural background, and preferred ways of communicating, while ensuring the patient is fully informed.
  • Able to work as part of a wider clinical and non-clinical team for the benefit of the patient and organization.
  • Management of patients with acute care needs.
  • Experience of managing patients with long-term conditions and/or frailty or within care homes.
  • Knowledge of health promotion strategies and comfortable giving lifestyle advice and signposting to agencies to support patients.
  • Knowledge of clinical governance issues in primary care.
  • Effective decision-making and negotiation skills.
  • Ability to work within own scope of practice and understand when to refer to GPs.
  • Ability to problem-solve while working under pressure.
  • Excellent organizational skills.
  • Accountability of own role and other roles in a nurse-led service.
  • IT skills (MS Office and EMISweb).
  • Flexibility to meet the needs of the business.
  • DBS required.
  • Meet the requirements and produce evidence for nurse revalidation.
  • Evidence of continuing professional development (CPD) commensurate with the role of an ANP or NP.
