A challenging and rewarding opportunity has arisen within the Medicine of the Elderly & Stroke Medicine team at RIE. As a key member of our team, you will play a vital role in supporting the Discharge Hub clinical and nursing teams, serving as the primary point of contact for all correspondence. Your responsibilities will include facilitating a seamless patient transition during discharge, collaborating with colleagues in the community, and ensuring a smooth patient pathway from an acute hospital setting.

Key Features of the Role:

  • Support the Discharge Hub clinical and nursing teams in a fast-paced environment
  • Field all correspondence as the first point of contact for the service
  • Liaise with colleagues in the community to ensure a seamless patient transition
  • Provide support for patients during discharge, ensuring a smooth and efficient process
  • Work collaboratively with the team to achieve shared goals and objectives
