Field Care Supervisor

About the Role
The Field Care Supervisor is a key position within our team, responsible for delivering high-quality care support services in the community. This role involves assessing and reviewing individual services, supervising and supporting care workers, and working closely with Care Coordinators to plan effective care worker rotas.
Key Responsibilities
- Carry out initial risk and needs assessments for individual service users in their homes or other settings.
- Work in partnership with service users, their families, carers, advocates, and healthcare professionals to ensure seamless delivery of holistic packages of care support.
- Shadow new care workers to ensure their competence and carry out on-site observations and evaluations of care worker practice.
- Participate in multi-disciplinary reviews of service users' needs and contribute to the development of individual care plans.
