**Job Description:**

We are seeking an experienced Intensive Care Worker to join our team in Tower Hamlets.

About the Role:

This is a challenging and rewarding opportunity for a skilled practitioner to provide high-quality care to individuals experiencing mental health crises. As an Intensive Care Worker, you will work closely with customers, their families, and external agencies to develop personalized care plans that promote independence and self-sufficiency.

Your Responsibilities:

  • Undertake key-working responsibilities for assigned caseloads
  • Conduct initial assessments and make decisions regarding access to crisis services
  • Develop and implement support plans tailored to individual needs
  • Provide emotional and practical support to empower customers to overcome their challenges
  • Liaise with healthcare professionals, community groups, and other stakeholders to ensure comprehensive care

Requirements:

To be successful in this role, you will need:

  • A minimum of 1 year's experience working with vulnerable adults
  • An NVQ Level 3 in Health and Social Care or equivalent qualification
  • Excellent communication and interpersonal skills
