Job Summary:
As a Social Worker in the Community Team, you will play a vital role in supporting adults with complex needs, including physical disabilities, mental health challenges, and learning disabilities. Your responsibilities will include assessing their needs, creating individualized care plans, and facilitating access to services that enhance well-being.
Key Responsibilities:
- Participate in assessments and care planning under the supervision of an experienced practitioner.
- Carry out mental capacity assessments in accordance with the Mental Capacity Act 2005.
- Make applications for Deprivation of Liberty Safeguards (DoLS) where appropriate, ensuring legal compliance and protecting individuals' rights.
- Manage a caseload, conducting robust assessments in partnership with service users, their carers, and multi-disciplinary partners where appropriate.
- Maintain accurate, clear, and timely records of assessments, interventions, and care plans, ensuring all documentation is in line with council policies and legal standards.
Requirements:
- Relevant social work qualification.
- 2 years of experience in adult social work, including assessment and care planning.
