Senior Physiotherapist Band 6
In East London NHS Foundation Trust, we are seeking a motivated and innovative Physiotherapy Specialist to join our dynamic Admission Avoidance and Discharge Service (AADS) team. This is an exciting opportunity for an enthusiastic physiotherapist to work at the forefront of integrated care, collaborating closely with multidisciplinary teams to deliver timely and effective physiotherapy interventions that prevent unnecessary hospital admissions.

The successful candidate will have a strong communication and collaboration skills, experience in community care, and a commitment to working across health, social care, and voluntary sectors boundaries. They will need to be a versatile and adaptable team player who is prepared to work across a 7-day rota. The salary for this role is £40,000 - £50,000 per annum, depending on experience.

Main Responsibilities:
  • To manage a defined caseload using evidence-based/client-centered principles to assess, plan, implement, and evaluate interventions, maintaining associated records.
  • To provide a high-standard physiotherapy service to the specialist caseload across various sites within Tower Hamlets as the service requires, focusing on admission avoidance and supporting timely hospital discharges.
  • Actively participate in the Homeless Project, providing drop-in sessions and delivering health promotion activities to empower homeless individuals to improve their mobility, foot health, and general well-being.
  • Engage in holistic care planning for vulnerable populations, addressing barriers to care and promoting self-management strategies.
  • To perform advanced physiotherapeutic assessments of patients with diverse presentations and complex physical and psychological conditions to provide a diagnosis and develop and deliver an individualized treatment program.
  • To provide leadership for more junior staff, including supervision of students on placement, as agreed with senior staff and deputies for Senior Physiotherapists in their absence as required.
  • To participate in CPD and develop skills and knowledge through participation in the in-service training scheme.
  • To undertake lone working when required.
  • To be able to work flexibly and participate in potential future 7-day working developments (when required).
