Clinical Nurse Practitioner

We are seeking an experienced Practice Nurse to provide exceptional nursing care and support to patients, assist doctors and other healthcare professionals, and play a key role in the efficient operation of the Practice.
Main Duties:- Manage chronic conditions such as asthma, COPD, and diabetes, including regular monitoring, patient education, and collaboration with multidisciplinary healthcare teams to optimise patient outcomes.
- Deliver a holistic approach to travel health by advising patients on vaccinations, medications, safe sex practices, food hygiene, and sun protection to ensure safe and informed travel.
- Administer childhood and adult vaccinations as per patient group directions, providing informed advice and ensuring adherence to immunisation protocols.
- Lead well person clinics and health promotion initiatives, offering lifestyle assessments and advice on diet, smoking cessation, alcohol reduction, and physical activity to enhance overall health and well-being.
- Administer vaccinations and offer comprehensive parental guidance and support in baby clinics.
- Conduct cervical cytology smear tests according to local and national guidelines, ensuring accurate documentation and patient care.
- Carry out venepuncture procedures following local protocols to facilitate diagnostic testing and patient care.
- Provide a holistic approach to wound management, including wound care 'Tier 2' lower limb, (ulcer care and Doppler) and implement care in line with current evidence-based guidelines.
- Conduct electrocardiograms (ECGs) to support the diagnosis and monitoring of cardiac conditions.
- Provide advice and administer contraception options, ensuring patient education and adherence to clinical guidelines.
- Safely and efficiently remove sutures, ensuring adherence to clinical protocols and patient comfort.
- Maintain awareness of statutory and local clinical protection procedures, including referral systems, and possess the ability to recognise signs and symptoms of child abuse, ensuring appropriate actions are taken to safeguard vulnerable patients.
- Safely manage the disposal of sharps and hazardous materials in compliance with health and safety regulations, ensuring a clean and safe environment for both staff and patients.
- Strictly follow infection control protocols to prevent the spread of infections and maintain a safe healthcare environment, adhering to all national and local guidelines.
- Recognise and manage anaphylaxis in accordance with current UK guidelines, providing prompt and effective treatment during emergencies, and perform cardiopulmonary resuscitation (CPR) as required, ensuring timely intervention in life-threatening situations.
- Follow established procedures for handling and cleaning body fluid spills, maintaining a safe and sanitary environment in line with health and safety standards.
- Obtain and document informed consent (verbal or written) in accordance with legal and ethical standards.
- Monitor and manage the maintenance of stock and equipment, including refrigeration and emergency equipment, ensuring that all resources are readily available and in good working order.
- Ensure the effective management of vaccine storage, including maintaining the cold chain and proper stock rotation, to uphold the integrity of all immunisation products.
- Maintain the highest standards of confidentiality regarding all patient and practice information, ensuring compliance with privacy regulations and safeguarding sensitive data.
- Ensure all consultations are documented in accordance with Nursing and Midwifery Council (NMC) guidelines, maintaining accurate and up-to-date patient records.
- Demonstrate a thorough understanding of national and local health guidelines, applying these standards to patient care to ensure high-quality and compliant practice.
- Undertake any other delegated duties appropriate to the post, contributing to the efficient running of the practice and ensuring the delivery of high-quality patient care.
- Contribute to the assessment of service needs.
- Assist patients to identify their health needs.
- Manage and organise individual patient consultations.
- Aware of identification and reporting procedures related to professional standards.
- To attend staff meetings (when appropriate).
- Ensure effective relationships and communications with the PHCT and other agencies.
- Support the Practice clinical governance agenda.
- Identify changes to clinical practice that are required to implement evidence-based guidelines.
- Identify personal development and training needs in conjunction with the manager.
- Participate in clinical supervision.
- Support others with their training and development needs.
- To participate in continuing education and maintain a contemporary level of professional knowledge and skills.
- Maintain good working relations.
- Take reasonable care of his/her own safety and that of other persons who may be affected by his/her act or omission.
- Co-operation with the Practice to ensure all members of the team adheres to statutory regulations/policies and codes of practice.
- Offer innovative ways of working and opportunities to facilitate learning.
- Facilitates a learning environment within the team.
- Maintains a caring environment through the support of colleagues.
- Maintain the highest standards of confidentiality regarding all patient and practice information, ensuring compliance with privacy regulations and safeguarding sensitive data.
- Information relating to patients, carers, colleagues, other healthcare workers or the business of the Practice may only be divulged to authorised persons in accordance with the Practice policies and procedures relating to confidentiality and the protection of personal and sensitive data.
