We are looking for a highly skilled Primary Care Pharmacist Lead to join our team in Doncaster East Primary Care Network (PCN). As a Clinical Network Pharmacist, you will play a key role in providing pharmacy services to patients and supporting the PCNs with regards to prescription and medication queries.

About the Role

This exciting opportunity is ideal for a motivated pharmacist who is passionate about delivering excellent services to patients. The role involves working as part of a multi-disciplinary team in a patient-facing role and taking responsibility for areas of chronic disease management within the practice.

You will provide primary support to general practice staff with regards to prescription and medication queries, support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing. You will also provide expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practice(s).

Key Responsibilities

Some of your key responsibilities will include:

  • Optimising the use of medicines
  • Supporting PCNs with regards to prescription and medication queries
  • Improving the safety and quality of prescribing
  • Providing support and education to patients on self-management and ensuring referral to appropriate healthcare professionals if required
  • Implementing and managing the repeat prescribing process for practice and patient requests

You will also have the opportunity to work with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare, and help manage workload.

Requirements

To be successful in this role, you must have:

  • A General Pharmaceutical Council registration
  • A Master's Degree in Pharmacy (MPharm) or equivalent
  • An Independent Prescriber qualification or willingness to complete
  • Membership of the Royal Pharmaceutical Society
  • Specialist knowledge acquired through post-graduate diploma level or equivalent training/experience gaining independent prescribing qualification

You should also have at least two years of post-qualification experience and an appreciation of the nature of GPs and general practices. An ability to communicate complex information in an understandable form to a variety of audiences is also essential.

This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020, and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.

The estimated salary for this role is £60,000 - £80,000 per annum, depending on experience. If you are successful, the pre-employment health check will require evidence of vaccination status or exemption, where appropriate, and your vaccination status will be retained as part of your employment health record.
