We are seeking skilled Supportive Care Practitioners to join our team at Jane Lewis Healthcare in North Wales. As a Supportive Care Practitioner, you will be responsible for providing person-centred care to individuals with learning disabilities, promoting their independence and dignity in a safe and supportive environment.
Responsibilities:- Deliver high-quality care to individuals with learning disabilities, prioritizing their needs and preferences.
- Assist with daily living tasks, including personal care, meal preparation, and supporting social engagement and activities.
- Develop and implement personalized care plans to meet individual needs and goals.
- Maintain accurate records and contribute to the wider support team's efforts to improve individual outcomes.
- A competitive salary range of £28,000 - £38,000 per annum, based on experience.
