About the Role

The Unity Surgery is seeking a highly skilled and experienced Practice Nurse to join our team. As a Practice Nurse, you will be responsible for providing high-quality patient care, working closely with our GPs and other healthcare professionals to deliver excellent healthcare services.

Key Responsibilities

  • Act as the Infection Prevention Control (IPC) lead and work to the IPC Policy
  • Develop, implement, and embed health promotion and well-being programmes
  • Implement and evaluate individual treatment plans for chronic disease patients
  • Identify, manage, and support patients at risk of developing long-term conditions
  • Provide routine nursing care to patients as required
  • Diagnose and manage acute and chronic conditions
  • Prescribe medication for patients in accordance with PGDs and own scope of practice
  • Review patient medications to enhance compliance
  • Provide guidance to patients on the use of prescribed and over-the-counter medications
  • Oversee the provision of wound care services to patients
  • Coordinate the provision of travel medicine services
  • Request pathology services as necessary
  • Process pathology results as required
  • Oversee the provision of chronic disease clinics
  • Maintain accurate clinical records
  • Ensure the appropriate maintenance and use of chronic disease registers
  • Develop, implement, embed, and participate in well-woman and well-man clinics
  • Chaperone patients where necessary
  • Assist GPs with minor surgery when required
  • Prioritize health issues and intervene appropriately
  • Support the team in dealing with clinical emergencies
  • Recognize, assess, and refer patients presenting with mental health needs
  • Implement vaccination programmes for adults and children
  • Support patients in the use of their prescribed medicines or over-the-counter medicines
  • Liaise with external services/agencies to ensure patient support
  • Delegate clinical responsibilities appropriately
  • Support the clinical team with safeguarding matters
  • Understand practice and local policies for substance abuse and addictive behavior
  • Deliver opportunistic health promotion

  • Registered nurse
  • Experience of working in a primary care environment
  • Experience of chronic disease management
  • Experience of working autonomously
  • Wound care/removal of sutures and staples
  • ECGs
  • Venepuncture
  • New patient medicals
  • Chaperone procedure
  • Requesting pathology tests and processing results
  • Diabetes
  • Hypertension
  • Asthma
  • Spirometry
  • CHD
  • Immunisations
  • Women's health
  • Understanding of evidence-based practice
  • Broad knowledge of clinical governance
  • Ability to record accurate clinical notes
  • Ability to work within own scope of practice
  • Understanding of infection prevention and control measures

Desirable Qualifications

  • Postgraduate diploma or degree in Chronic Disease Management
  • Nurse prescriber
  • Experience of leading a nursing team in a primary care environment
  • Experience of working as a practice nurse or community nurse
