About the Role

We are seeking an experienced and dedicated individual to fill the position of Community Practitioner in our team. As a Community Practitioner, you will play a vital role in supporting adults across a wide geographical area.

Job Overview

In this role, you will work closely with adults with care and support needs, their families, and carers. Your primary goal is to promote independence, wellbeing, and informed choices for individuals.

You will deliver person-centred care and support tailored to individual goals and preferences. This includes conducting comprehensive assessments, creating care plans, and reviewing them regularly to meet personal outcomes.

A key aspect of this role is building strong community links and partnerships to provide effective support networks. You will also be responsible for managing crisis situations and leading safeguarding enquiries.

To excel in this position, you must possess excellent communication, mediation, and negotiation skills. Strong IT and administrative capabilities are also essential, as well as the ability to manage workloads independently and as part of a team.

Requirements
  • Degree or Diploma in a relevant field (e.g., Social Work, Nursing, or Occupational Therapy)
  • Up-to-date registration with your professional body
  • Competence at the new practitioner level of the Social Care Capabilities Framework
Desirable Qualifications
  • Post-qualification experience in social care, health, or related fields
  • Expertise in risk management, safeguarding enquiries, and supporting individuals with medium to high-level complex needs
  • Proven ability to work collaboratively in multi-agency environments
Estimated Salary: £35,000 - £45,000 per annum
