We are seeking a highly skilled and compassionate Care Coordinator to join our Hospital team in Newham, London. As a key member of our multidisciplinary team, you will be responsible for conducting comprehensive assessments and developing individualized care plans that cater to the unique needs of our patients.

Main Responsibilities:
  • Conduct thorough risk assessments and develop effective care strategies
  • Collaborate with medical staff and other healthcare professionals to ensure seamless patient care
  • Provide emotional support and advocacy to patients navigating the healthcare system
  • Work closely with community services and stakeholders to promote patient well-being and facilitate safe discharges

The successful candidate will have a degree-level qualification in Social Work or equivalent and at least 2 years of post-qualified experience as a social worker. A full UK driving licence is also essential.
