Home Independence Advisor

We are looking for a dedicated and caring Home Independence Advisor to join our Reablement team in Cheshire West and Chester. As a key member of our adult social care service, you will support individuals to achieve their full potential and live independently in their own homes.
About the Role:
As a Home Independence Advisor, you will work closely with our team to deliver high-quality care and support services that meet the unique needs of each individual. You will assess and develop personalized plans to support people to regain their independence, confidence, and skills to maintain their health and well-being.
Key Responsibilities:
• Assess the needs of individuals and develop personalized plans to support them to regain their independence and confidence.
• Work collaboratively with our team to deliver high-quality care and support services that meet the unique needs of each individual.
• Provide guidance and support to service users and their families to enable them to make informed decisions about their care and support.
• Manage medication and support meal preparations as required.
Requirements:
• Previous experience in a care setting is not essential as full training is provided throughout your career.
• Strong communication and interpersonal skills to build effective relationships with service users, families, and colleagues.
• Ability to work independently and as part of a team to achieve outstanding results.
• Commitment to providing person-centred care and support that respects the dignity and autonomy of each individual.
• Enhanced rates paid for Bank Holidays, Evening, and Weekend working.

The estimated salary for this role is £22,000 - £28,000 per annum, depending on experience.
