Pharmacist in Medicines Optimisation

About this role

This position is responsible for delivering medicines optimisation services within our practice. As a Clinical Pharmacist, you will work closely with our team to develop and manage medicines optimisation services, ensuring that patients receive the best possible care. You will be responsible for providing clinical leadership on medicines optimisation and quality improvement, and will work closely with our team of pharmacists and pharmacy technicians to achieve this goal.

Key responsibilities

• Provide clinical leadership on medicines optimisation and quality improvement
• Develop and manage medicines optimisation services within our practice
• Work closely with our team of pharmacists and pharmacy technicians to achieve our goals
• Provide expertise in clinical medicines advice and address both public and social care needs of patients in the GP practice
• Ensure that the practice integrates with community and hospital pharmacy to improve patient outcomes and better access to healthcare

About our team

We are a GP Federation covering 16 practices, serving over 130,000 patients. We have a team of 12 pharmacists, including 3 senior pharmacists, and 6 pharmacy technicians.
