We are seeking a highly skilled and dedicated Pharmacist Outpatient Specialist to join our team at Epsom and St Helier University Hospitals NHS Trust. As a key member of our outpatient department, you will play a vital role in providing high-quality pharmaceutical care to our patients.

Main Duties and Responsibilities
  • Deliver high-quality pharmaceutical care to patients in the outpatient department, working closely with healthcare professionals to ensure seamless care.
  • Conduct medication reviews, provide medication therapy management, and optimize patient treatment plans.
  • Collaborate with multidisciplinary teams to ensure effective communication and coordination of patient care.
  • Participate in quality improvement initiatives to enhance patient outcomes and departmental performance.
  • Develop and maintain strong relationships with patients, families, and healthcare professionals to ensure excellent patient experience.
Person Specification
  • Registered Pharmacist with current registration with the General Pharmaceutical Council (GPhC).
  • Proven experience in outpatient pharmacy practice, with a strong understanding of pharmaceutical care principles and practices.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Strong problem-solving and analytical skills, with the ability to prioritize tasks and manage multiple projects simultaneously.
  • Ability to work independently and as part of a team, with a flexible approach to work and a willingness to adapt to changing priorities.
