Job Opportunity for a PCN Clinician


Service Care Solutions is seeking a skilled PCN Clinician to join our team in a Primary Care setting.


As a PCN Clinician, you will work autonomously within the community, utilizing your enhanced clinical assessment and treatment skills to provide first-point contact for patients presenting with undifferentiated, undiagnosed problems. You will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within their own homes.


You will use a medical model to assess, diagnose, and prescribe for acute medical conditions, while also employing a holistic approach to consider methods and interventions to prevent recurrence of problems and complete referrals to appropriate members of the wider multidisciplinary team.


  • Contract Type: Fixed Term (14 months)
  • Working Hours: Full Time - 37.5 hours per week
  • Support Network: Excellent support network available
  • Working Schedule: Monday to Friday
