Job Summary
We are looking for a skilled and compassionate Practice Nurse to work alongside our clinical staff and provide exceptional care to our patients. The successful candidate will have a strong background in primary care and be able to carry out a range of clinical duties, including wound care, chronic disease management, and immunization services.
Key Responsibilities
- Provide routine nursing care to patients, including wound care and chronic disease management
- Carry out immunization services, including vaccinations and travel medicine
- Request and process pathology services as necessary
- Maintain accurate clinical records and ensure effective use of read codes
- Support patients in the use of their prescribed medications and provide health promotion services
- Work collaboratively with the clinical team to provide high-quality patient care
Person Specification
We are looking for a Practice Nurse with:
- Primary care experience, including wound care and chronic disease management
- Ability to carry out immunization services, including vaccinations and travel medicine
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
