We are seeking an experienced and motivated Therapy Technician to join our Early Supported Discharge Team. As a key member of our team, you will play a vital role in supporting patients in their transition from hospital to home.
Main Responsibilities- Work as a Therapy Technician in hospital and patient homes, providing independent support under the direction of qualified Therapists.
- Support patients to return home promptly and safely, ensuring a smooth transfer to the community and maximizing their independence at home.
- Assist patients in their rehabilitation and mobility, transfers, personal care, meal preparation, and activities of daily living.
- Work under Therapists' direction to assist patients in achieving person-centered goals.
- Participate in weekend rotas and support Therapists in making assessments and onward referrals to community services for care and ongoing support and rehabilitation.
- Implement and monitor treatment programs for patients, within the framework laid down by Occupational Therapists and Physiotherapists, and make assessments of progress.
- Assist in the implementation, evaluation, and modification of Occupational Therapy and Physiotherapy.
- Participate in providing a high-quality, effective, and efficient Occupational Therapy and Physiotherapy service.
- Accurately record interventions in a timely manner.
- Work autonomously, assessing risk, and judge when senior support is required.
