About the Role

We are seeking a dedicated and compassionate Support Worker to join our Health and Local Crisis Response service. As a Support Worker, you will play a vital role in providing practical and emotional support to individuals in need, enabling them to live healthier and more fulfilling lives.

Key Responsibilities

  • Provide reactive, focused, and flexible support to adults following a hospital stay to enable early discharge.
  • Respond to referrals from Clinicians and Community Health Professionals.
  • Assess service users' needs, complete and follow a support plan for the individual.
  • Support service users in the weeks after hospital discharge or to prevent readmission.
  • Provide transportation to hospital appointments and assist with daily tasks such as shopping and prescription collection.

Requirements

  • A full UK manual driving licence.
  • A good knowledge of services provided by the NHS and Social Care.
  • The ability to make things great and improve service quality for the benefit of users.
  • Professionalism and the ability to deal with queries in a diplomatic and confidential manner.
  • Flexibility and the ability to work hours outside of the norm.
