About the Role

We are seeking a highly skilled Therapy Support Worker to join our Community Health and Social Care team in Barnstaple, North Devon. As a key member of our team, you will work closely with registered therapy team members to support patients with a range of interventions, including mobility and home exercise programmes, small equipment aids, and functional tasks around the home.

Key Responsibilities

  • Support patients with a range of disabilities and acute or long-term conditions to reach their optimum level of function and independence.
  • Work with patients and caregivers to achieve goals that are important to the patient, such as being able to walk with no assistance around their home or being able to make a snack in the kitchen while experiencing reduced strength.
  • Document all contacts and maintain accurate records of patient interactions.
  • Communicate regularly with other health and social care professionals and wider partners to ensure seamless care delivery.

Requirements

  • Experience of working in a care or rehabilitation setting.
  • Good communication skills, a proactive and practical approach, and good organisational skills.
  • Ability to work independently and as part of a team.
