Job Title: Band 7 District Nurse Team Leader

We are seeking a highly skilled and experienced Registered Nurse with a Specialist Practitioner qualification to join our Integrated Care Team as a Band 7 District Nurse Team Leader.

Key Responsibilities:
  • Lead a team of Community Nurses and Health Care Support Workers, providing supervision and appraisal.
  • Manage a locality caseload of patients, assessing, managing, planning, and delivering care.
  • Develop and implement care plans, ensuring safe and effective patient outcomes.
  • Collaborate with other healthcare professionals to ensure seamless care delivery.
  • Contribute to the development of the team and the organization, sharing expertise and best practices.
Requirements:
  • Registered Nurse with a Specialist Practitioner qualification.
  • Experience in community nursing, with a focus on leadership and management.
  • Excellent clinical and communication skills, with the ability to work effectively in a team.
  • Valid UK driving license, with the ability to travel between sites.
  • Non-medical or community nursing prescribing skills.
