Healthcare Assistant Practitioner

About the Job:
The Healthcare Assistant Practitioner will work as part of a friendly, small multi-disciplinary team in the Neuro Rehabilitation Therapy treatment programme for patients with a diagnosis of Multiple Sclerosis (MS). The objective is to promote self-management of their condition, aiming to maintain the independence of people living with MS in the community in South Warwickshire.
Main Responsibilities:
- Deliver high-quality, caring, and compassionate patient care.
- Undertake assessment, implementation, and evaluation of patient care needs for a defined group of patients under the indirect supervision of a registered professional in their specialist area.
- Work as part of a team committed to delivering and improving a high standard of nursing care within South Warwickshire University NHS Foundation Trust.

Key Requirements:
- Obtained an educational level of GCSE level A-C English and Maths or equivalent.
- Minimum Foundation Degree in Health and Social Care or equivalent higher apprenticeship.
