The East Lynne Medical Centre is seeking a highly skilled and experienced Cancer Care Nurse to join our team. As a key member of our Primary Care Network, you will play a vital role in improving the health and wellbeing of patients within our network through improved screening, holistic assessment, and proactive management of cancer conditions.
Key Responsibilities- Identify and coordinate the implementation and evaluation of treatment/interventions and care for patients with complex needs.
- Assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
- Liaise with wider multidisciplinary teams and support the management of both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
- Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
- Work with patients to support compliance with and adherence to prescribed treatments.
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Support the completion of Advanced care planning where appropriate, including My care choice register, RESPECT forms and PPC documentation.
- Support practices with cancer patients that may rapidly decline and facilitate the right services to meet the patients needs.
- Work with other multidisciplinary teams to provide a holistic approach to the management of individuals needs, for example referral to social prescribers or occupational therapists.
- Ensure accurate and legible notes of all consultations and treatments are recorded promptly in the patients notes on SystmOne/EMIS.
- Maintain infection control standards in varied and challenging environments in line with the organisations infection control policy and procedural guidance.
- Ensure the safety of patients, relatives, and colleagues and to report all incidents and near misses in line with the risk management policy.
- Maintain up-to-date knowledge of safeguarding adult and children guidance to ensure that those at risk of abuse/suffering abuse are identified and the necessary process to protect them is followed.
- Meet responsibilities as an employee as set out in the organisations corporate and clinical policies.
- Take responsibility for own continuing professional developmental learning, participate in self-evaluation and reflection.
- Lead in the development of this role, focusing on the needs of the practice population. Working with other stakeholders to educate our population re cancer screening programmes and improving uptake of these.
- Develop the robust cancer care reviews within the practices ensuring the same quality is being delivered across our population. Including development of resource pack for all to refer to.
- Leading on Patient education events for health promotion.
- Be a role model for PCN and practice staff, promoting effective cancer care and supporting new ways of working.
- Take a lead on Cancer care in the wider development of the PCN and local and National agenda.
- Assist in clinical audit processes, supporting demonstration and analysis of Cancer care outcomes.
- Engage and evaluate peer, patient, and service evaluations.
- Use research and evidence-based practice to share ideas for improvements and efficiencies.
- Alert the Clinical Lead/other team members to issues of quality and risk in the care of patients.
- Ensure own actions are high quality and consistent with clinical governance.
- Practice in accordance with agreed standards of care.
- Enable patients to access appropriate professionals in the team.
- Ensure stock items under your control are replenished or reported, as appropriate.
- Be aware of practice-specific policies and procedures.
- Demonstrate effective time and work-load management.
- Act in ways that recognises and respects patients choices and wishes.
- Respect the privacy, dignity, needs and beliefs of patients and carers.
- Understand basic legal and communication issues regarding abuse, violence, vulnerable adults, substance abuse and addictive behaviours.
- Record daily activities and information undertaken in an accurate and timely fashion using IT, as appropriate.
- Maintain accurate and contemporaneous electronic patient records on SystmOne/EMIS.
- Maintain the confidentiality of sensitive information relating to patients, relatives, staff and the practice.
- Omit patient-identifiable information, as appropriate when sharing data.
