Job Title: Social Care Coordinator - Older People Services

About the Role:
We are seeking an experienced Social Care Coordinator to join our team in Older People Community Services. As a Social Care Coordinator, you will work under the direction of a social worker to deliver a person-centered social care service to older people, their families, and carers.

Key Responsibilities:
• Work with and under the direction of the social worker to deliver a person-centered social care service to older people, their families, and carers.
• Carry out a range of activities and interventions in accordance with individual care plans to maximize service users' potential, promote community integration, reduce isolation, and promote confidence.
• Contribute to the monitoring of the effectiveness of domiciliary care packages and other services in place to support service users to live in their own home.
• Liaise with the multidisciplinary team, including district nursing, GPs, and hospital staff, regarding the needs of service users.
• Support the social worker to comply with internal administrative processes.

Requirements:
• NISCC Registration
• Enhanced Access NI cleared
• 12 months previous Social Work Assistant or Social Care Coordinator experience
• Social Work Degree, BTEC, or NVQ Level 3 Health & Social Care or equivalent qualifications
• Willingness to undergo an Occupational Health check, including submission of immunization records as required
• Previous experience working with older people and/or people with physical health disabilities highly desirable
• Driving license highly desirable although not essential for all roles
• Knowledge of hospital and community services highly desirable
• Effective communicator
• Ability to prioritize and manage a varied caseload
