About the Role:

Key Responsibilities:

  • Assess and monitor patient conditions, arranging diagnostic tests as needed.

  • Develop and implement care plans in collaboration with patients, families, and healthcare teams.
  • Provide education and support to patients and their families, promoting healthy behaviors and disease management.
  • Work collaboratively with multidisciplinary teams to ensure seamless patient care.

Person Specification:

Essential:

  • Degree/Diploma in Nursing, with current NMC registration.
  • A minimum of 6 months paid experience in a community environment within the last 2 years.
  • A valid UK driving license and access to a car.
