Job Overview

We are seeking an experienced Lead Community Nurse Practitioner to join our East and South Cheshire Community Learning Disability Team. This is an exciting opportunity to work with a highly motivated team, delivering high-quality care to adults with learning disabilities in the community.

The successful candidate will have a strong background in nursing, with experience working with adults with complex needs, including challenging behavior, epilepsy, mental health problems, and continuing healthcare.

As a Lead Community Nurse Practitioner, you will be responsible for managing a defined caseload of adult service users with learning disabilities, providing holistic assessments, developing personalized care plans, and delivering training to other staff, service users, and carers.

You will also have the opportunity to lead quality improvement initiatives and contribute to the development of the team's services.

Main Responsibilities
  • Manage a caseload of adult service users with learning disabilities, providing holistic assessments and developing personalized care plans
  • Deliver training to other staff, service users, and carers on various topics related to learning disabilities and community nursing
  • Participate in service developments and contribute to the improvement of the team's services
  • Collaborate with other healthcare professionals to ensure seamless care delivery
Requirements
  • Registered Mental Health Nurse (RMN) or Registered Learning Disability Nurse (RNLD)
  • Post-registration module training in specialist areas, such as physical health in learning disabilities or epilepsy
  • Experience working with adults with complex needs, including challenging behavior, epilepsy, mental health problems, and continuing healthcare
  • Demonstrated commitment to ongoing learning and development
Benefits
