We are seeking a compassionate and experienced Young Person's Support Worker to join our team in Lancashire. As a key member of our team, you will provide person-centred support to young people in our Supported Accommodation projects, ensuring their continued safe provision of accommodation and well-being.

Your role will involve undertaking needs assessments, providing advice, guidance and support to young people, and working collaboratively with a multidisciplinary team. You will also be responsible for completing support reviews and SMART goals, ensuring the highest standard of service delivery.

You will work closely with young customers, group employees, and outside agencies to deliver a holistic service that meets the needs of our customers. Your expertise in safeguarding and risk management, planning transition and move-on, and team working will be essential in this role.

As a successful candidate, you will have experience working with individuals 16+, a customer-facing role, and excellent communication skills. You will be a confident I.T. user, able to influence positively when required, and possess local knowledge to share with our customers.
