Community Health Practitioner

We are seeking a dedicated and experienced Practitioner to join our team in providing community-based health services to children and young people across Essex.

Your primary role will be to work closely with families, schools, and other partner agencies to identify and respond to the needs of vulnerable young people, delivering targeted interventions and groups to meet their needs, and contributing to the design and delivery of health promotion campaigns.

You will work collaboratively with service users, colleagues, and external bodies to ensure that children and young people receive the right support at the right time, drawing on your expertise in areas such as emotional well-being, exploitation, and risk-taking behavior.

In addition to your clinical expertise, you will be responsible for assessing risk, managing caseloads, and developing interventions to improve health and wellbeing outcomes. You will also contribute to the development of policy and procedure, ensuring that our practice is aligned with national and local guidelines.
