Job Summary

We are seeking a highly skilled and experienced nurse or physiotherapist to join our innovative COPD team as a Community COPD Nurse/Physiotherapist. This is a 1-year fixed-term contract to cover maternity leave.

Key Responsibilities
  • Proactively manage a caseload of patients requiring complex case management to prevent hospital admissions and improve patient health outcomes.
  • Work autonomously, monitoring and managing patients on the caseload, and providing expert care to COPD patients registered with a Greenwich GP.
  • Utilize advanced clinical skills and expert knowledge to plan, implement, monitor, and review individualized patient-directed programs of care, including health promotion interventions.
  • Develop risk management plans to enable individuals to remain safely within their homes and provide individuals and their carers with necessary education to plan for unavoidable disease progression and self-manage as far as possible.
  • Manage a caseload of patients with COPD, practicing at an advanced level, demonstrating an in-depth knowledge of all aspects of the management of COPD that is evidence-based.
Requirements
  • Degree/diploma in nursing/physiotherapy
  • 4 years of experience
  • Demonstrate varied communication skills
  • Access to care
Desirable Qualifications
  • Postgraduate modules in respiratory
  • Community experience
  • Delivery of self-management
