Job Overview

The Greater Manchester Mental Health (GMMH) Foundation Trust is seeking a dynamic and innovative individual to join their Home Engagement and Rehabilitation Team as a Care Coordinator. This exciting opportunity is part of the Trust's commitment to providing high-quality, recovery-focused care to individuals living with enduring mental health problems.

Main Responsibilities

The successful candidate will play a central role in the Multi-Disciplinary Team (MDT) to ensure the delivery of high-quality care to service users in the community. Key responsibilities include:

  • Coordinating a small caseload of service users, working creatively to support them in identifying their goals and coordinating timely interventions to achieve these.
  • Maintaining a recovery focus and demonstrating skill and sensitivity in assessing vulnerable service users with higher levels of need.
  • Organizing and supervising recovery workers within the team, responding to crises as necessary, and delegating tasks to colleagues appropriately.
  • Playing a key role in the development of the service, assisting the leadership team in day-to-day operations, and contributing to the provision of a service sensitive to the needs of service users and colleagues.

Person Specification

The ideal candidate will possess:

  • Professional qualification and registration
  • Experience of working in a rehab setting or community mental health setting
  • Experience of working with people with complex needs and co-morbidities
  • MDT working experience
  • Ability to assess, plan, and evaluate care
