Salary: £25,235 per annum (pro rata for part-time posts)

Job Description

This role involves delivering one-to-one crisis support to clients face-to-face or over the telephone, working closely with our multidisciplinary team to ensure that clients receive comprehensive support services.

You will be responsible for managing referrals, booking appointments, conducting follow-up calls after crisis sessions, making onward referrals, and signposting where necessary.

Additionally, you will participate in shift handovers and team meetings, offer outreach sessions within the community, liaise with professionals and organizations across the district, and ensure that client notes and information are recorded accurately and in a timely manner.

Main Duties:

  • Deliver one-to-one support sessions to people in mental health crisis
  • Manage referrals, book appointments, and conduct follow-up calls after crisis sessions
  • Make onward referrals and signpost where necessary
  • Participate in shift handovers and team meetings
  • Offer outreach sessions within the community
  • Liaise with professionals and organizations across the district
