Community Healthcare Coordinator

We are looking for an experienced Registered Nurse to fill the role of Community Healthcare Coordinator. This position offers a competitive salary, excellent benefits, and opportunities for career growth and development.
Salary: £31,500 - £35,500 per annum (pro-rata), dependent on experience
Responsibilities:
- Deliver high-quality patient care, prioritising individual needs and preferences
- Collaborate with the Registered Manager to review and assess referrals, ensuring timely interventions and effective solutions
- Provide clinical leadership and guidance to healthcare workers, supporting their professional development and competence
- Work closely with the Community Care Nurse Manager to drive continuous improvement in community care services
- Develop and maintain comprehensive care plans, risk assessments, and associated documentation, adhering to best practices and regulatory requirements
- Support and facilitate debriefing sessions for service users and carers following critical incidents or difficult situations
