Job Summary: We are seeking a skilled Practitioner in Care Support to join our team at Four Seasons Health Care Group. As a key member of our care team, you will be responsible for delivering high-quality person-centred care to our residents, working closely with our registered nurses to ensure the best possible outcomes. In this role, you will have the opportunity to make a real difference in the lives of others, providing support and companionship to those who need it most.

Responsibilities:
  • Collaborate with the registered Nurse to ensure that all residents receive quality medical and personal care.
  • Support the Home Manager in implementing policies, procedures, and business objectives.
  • Develop and maintain effective relationships with residents, their relatives, and visitors, as well as health and social care professionals.
  • Provide guidance and support to colleagues, promoting a culture of excellence and continuous improvement.
  • Stay up-to-date with industry developments and best practices, applying this knowledge to improve our care services.

Requirements:
  • Proven experience in a care setting, able to evidence full competence.
  • Qualifications in health and social care, or working towards a higher-level qualification.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with residents, colleagues, and external partners.
  • A commitment to ongoing professional development, staying current with industry trends and best practices.
  • A passion for delivering high-quality care, with a focus on person-centred approaches and individualised support.
