Job Overview

About the Role

We are looking for a talented individual to support our backlog of annual reviews and phase transfer 0-25. The successful candidate will have a thorough understanding of the required process when amending EHCPs and experience in case management.

Key Responsibilities
  • Process annual reviews and phase transfer 0-25 efficiently
  • Have a person-centred approach and provide excellent support to service users
  • Work closely with the team to ensure seamless transitions
Required Skills and Qualifications
  • Experience in processing annual reviews
  • Knowledge of the phase transfer process
