About the Role:

The Young Carer Assessor Practitioner will play a crucial role in identifying the specific needs of young carers and coordinating support through commissioned services at Wiltshire Council's Integrated Front Door service.

Main Responsibilities:

  • Conduct assessments of young carers under 18 who are caring for loved ones facing health challenges or substance misuse.
  • Collaborate with families and professionals to develop support plans that meet the unique needs of young carers.
  • Work closely with local authorities and service providers to ensure access to commissioned services.

Required Qualifications and Experience:

  • Degree in Social Work, Child Care, or a related field.
  • Relevant experience working with vulnerable children and young people.
  • Excellent communication and assessment skills.
  • Able to work independently and as part of a team.

  • An estimated salary range of £25,000 - £30,000 per annum.
