At St Columba's Hospice, we are redefining hospice care to provide the best possible experience for our patients and their families. As a member of our Care at Home Team, you will have the opportunity to shape and develop our service, providing practical, social, and emotional support to palliative patients within their own homes.

The role involves working closely with our registered Nursing staff Lead and Coordinator to deliver a person-centred approach in preventing hospital admissions or facilitating discharge from inpatient care. You will also be responsible for providing respite support to families and carers, promoting independence through enabling, empowering, and educating patients and families.

We are looking for an experienced Community Clinical Support Worker to join our team in North Edinburgh and East Lothian. The successful candidate will have a full UK driving license and use of a car. Key responsibilities:

  • Deliver person-centred care to patients and their families
  • Work collaboratively with multidisciplinary teams to achieve seamless care transitions

Benefits:

  • Competitive salary: £27,933 (pro rata) rising to £28,998 subject to satisfactory performance review
  • Unsocial Hours Shift Allowance + Mileage Allowance + Pension + Benefits
  • Hospice Band: 3
  • Hours: 36 hours per week - 12 hour day shifts / 3 days out of 7 / no set pattern
