About You
We are looking for a highly motivated and experienced mental health practitioner who can demonstrate a strong commitment to delivering high-quality care and services. As a Senior Mental Health Practitioner, you will be responsible for assessing and planning individualized care packages, working collaboratively with colleagues to develop and implement care plans, and promoting a culture of excellence within the care home.

Your Responsibilities
- Assess and plan individualized care packages for residents
- Develop and implement care plans that promote recovery and well-being
- Collaborate with colleagues to deliver high-quality care and services
- Identify and address areas for improvement in care delivery and staff performance
- Participate in quality assurance and risk management activities

Your Requirements
- A relevant mental health qualification and registration
- Proven experience in a senior practitioner or leadership role
- Strong assessment and planning skills
- Ability to communicate effectively with colleagues, residents, and families
- Passion for delivering high-quality care and services
