Job Description

This is an exciting opportunity for a Clinical Development Specialist to join and assist with the development of the Hospital at Home service, alongside virtual wards specialities within Older Persons Services.

The role will include assessments, diagnostics and implementing a treatment plan whereby patients' conditions and plan of care can be met within their own home or within community settings to prevent them from remaining within the acute settings when deemed appropriate.

Key Responsibilities:

  • Conversant in older persons Advanced practice developments at national/local levels.
  • Legal/professional/ethical issues of advanced practice.

  • Demonstrable experience of working independently at a Band 7 Clinical role within relevant specialty.
  • Developing policies & protocols.
  • Non-Medical Prescribing.
  • Excellent interpersonal and communication skills, using a range of methods, e.g. verbal, written, record keeping, complex report writing.
  • The use of a car for transportation to visit patients at home is essential, as well as the willingness to obtain business car insurance.
  • A commitment to improving patient services through an ability to sustain a clear performance focus on achieving demanding goals.

About the Role

We are looking for a highly skilled and experienced Hospital at Home Service Coordinator to join our team and contribute to the development of our Hospital at Home service. As a key member of our team, you will work closely with our clinical staff to provide high-quality care to patients in their own homes.

Requirements

  • MUST have a strong understanding of Advanced Clinical Practice principles and practices.
  • MUST have excellent communication and interpersonal skills.
  • MUST be able to work effectively in a fast-paced environment.

What We Offer

We offer a competitive salary of £43,041 - £51,039 per annum, depending on experience, plus a range of benefits including pension scheme, annual leave, and professional development opportunities.
