Job Summary

We are seeking a highly motivated and compassionate Support Worker to join our team at North Yorkshire County Council. As a Support Worker, you will play a vital role in providing care and support to adults with learning and physical disabilities, enabling them to live independently in the community.

Key Responsibilities
  1. Provide person-centered care and support to individuals with learning and physical disabilities, promoting their independence and well-being.
  2. Develop and maintain positive relationships with service users, their families, and other stakeholders.
  3. Work collaboratively with other healthcare professionals to ensure seamless care and support.
  4. Assist service users with daily living tasks, such as personal care, meal preparation, and household chores.
  5. Support service users to access community activities, such as education, employment, and leisure activities.
  6. Monitor and record service user progress, identifying areas for improvement and developing strategies to address these.
  7. Participate in staff meetings and training sessions to stay up-to-date with best practices and policies.
  8. Contribute to the development and implementation of individual support plans, ensuring they are tailored to meet the unique needs of each service user.
Requirements
  • Diploma in Health and Social Care or equivalent.
  • Experience working with adults with learning and physical disabilities.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work varied shifts, including evenings and weekends.
  • Valid DBS check.
