About the Role

We are seeking a highly motivated and enthusiastic individual to join our integrated community nursing team as a Healthcare Assistant. This is an exciting opportunity to work with a recognized finalist in the 2023 Nursing Times Awards.

Key Responsibilities

  • Work with a registered community nurse on twilight duty for housebound patients in their own homes across Surrey Downs Health & Care localities.
  • Deliver clinical interventions as delegated by senior team members, following personalized care plans.
  • Provide personal, social, and emotional support to patients, promoting their health and well-being.
  • Maintain accurate and clear record keeping, participating in clerical duties as required by the team.
  • Assist with administrative tasks, taking and recording telephone messages, and responding appropriately.

Requirements

  • Previous experience of caring for patients.
  • Ability to work unsupervised and show initiative.
  • Good interpersonal skills and basic numeracy skills.
  • Reliable and a car driver with a clean UK license.
