About the Role
We are seeking an experienced Support Professional to join our team as a Community Care Specialist. As a key member of our Home First Team, you will play a vital role in supporting patients to return home following a hospital admission and facilitating early supported discharge from hospital.
Key Responsibilities
- Provide personal care and rehabilitation to support patients to return home
- Facilitate early supported discharge from hospital through access to health and care needs assessment at home
- Work collaboratively with the multidisciplinary team to ensure seamless care delivery
- Develop and maintain effective relationships with patients, families, and carers
Requirements
- Full UK Driving Licence
- Sound communication and interpersonal skills (verbal and written)
- Ability to organise and prioritise own work, moderate IT skills, ability to manage, and ability to prioritise care delivery in a professional manner
