Home Care Nurse RN LPN Job Description

Responsibilities:
  1. Provide high-quality patient care services in the comfort of clients' homes
  2. Conduct thorough assessments of clients' needs and develop personalized care plans
  3. Supervise and support our home health aide staff in the field to ensure exceptional care and service
  4. Collaborate with other healthcare professionals to ensure seamless care coordination
Requirements:
  1. Current RN or LPN licensure in the state of Massachusetts
  2. Strong clinical and computer competence
  3. Excellent communication and interpersonal skills
  4. Able to work independently and as part of a team
  5. Ability to problem-solve and motivate others
