Job Summary

We are looking for a highly motivated individual with a passion for primary healthcare to provide exceptional care to our patients. As a Band 6 Practice Nurse, you will work in a multidisciplinary team to deliver high-quality nursing care to the Medical Treatment Facility population.

About the Role

This is a fixed-term appointment ending on 28/03/2025, offering a competitive salary of £42,683 - £60,441 per annum, depending on experience. You will be responsible for working unsupervised, providing nursing care to patients with complex health needs, identifying, assessing, and diagnosing patients with minor injuries, illnesses, and specific diseases within your level of competence.

Key Responsibilities

  • Provide safe, evidence-based, cost-effective, and individualized patient care.
  • Manage the care of patients with chronic illnesses, including Asthma, Diabetes, Chronic Heart Disease (CHD), and hypertension, ensuring continued maintenance of the Chronic Disease Register.
  • Undertake technical nursing treatments and procedures for patients with complex/specialist healthcare needs, such as wound care, sutures removal, Electrocardiograms (ECGs), Cytology, Audiometry, Ear Care.
  • Coordinate the development of plans to meet the health needs of patients, including diagnostic tests, treatment plans, health promotion, and prevention.
  • Support the development and delivery of specialist services, such as long-term disease management (Diabetes, Asthma, Chronic Obstructive Pulmonary Disease, Hypertension), acting as a regional resource for advice and regional clinics.

Requirements

To be successful in this role, you must have:

  • A professional and specialist knowledge in primary healthcare acquired through postgraduate degree/diploma or actively working towards this, demonstrating equivalent levels of competence and experience in clinical assessment and diagnosis.
  • Learning and Assessing in Practice Qualification or equivalent practice assessors training.
  • Quality improvement methodology or willingness to undertake training.
  • Basic Life Support (BLS) Anaphylaxis, Automated External Defibrillation (AED) Infection Prevention and Control (IPC) Safeguarding certifications.
  • Level 3 (Children and Adult depending on population at risk) or willingness to undertake training.
  • Registered non-medical prescriber or willingness to undertake training.
