We are seeking a skilled and dedicated Social Care Worker to join our team at Care at Home Services in Glasgow.
About the RoleAs a Social Care Worker, you will be responsible for undertaking initial assessments for care planning and reviewing existing service users. You will work in partnership with Occupational Therapists, Area Operations Managers, and local internal and external agencies to achieve the best outcomes for service users.
Key Responsibilities- Undertake initial assessments for care planning and review existing service users
- Work in partnership with Occupational Therapists, Area Operations Managers, and local internal and external agencies
- Ensure delivery meets National Care Standards and Glasgow City HSCP service objectives
- Participate in a Duty Rota for Care at Home
To succeed in this role, you will be a confident, self-motivated, and driven individual with a flexible approach. You will have excellent interpersonal skills, the ability to communicate effectively with service users and their families, and the ability to navigate and record on Care First.
