About the Role

This is a casual opportunity, with shift ties from 08:00 to 16:00 hours. You can expect to work a variable number of hours, ranging from 0 to 37.5 per week. Many careers with us begin with casual employment and lead to permanent or temporary opportunities.

Responsibilities
  • Provide comprehensive assessment, care planning, and treatment coordination for designated clients and their families.
  • Collaborate with other healthcare providers and the multidisciplinary team to provide direct psychosocial care, counseling, system navigation, and advocacy.
  • Foster client self-management and linkage to relevant specialized complex care resources.
